How to use Google maps in the Contact Form

I want to show a google map from our location in the head of our contact form. How can I get a shortcode oder something else which i can use.

All have ten rows with one Visual Editor Widget per row. For some reason when I get to add the fifth row I can add the visual editor ok and add text.
