I have been trying to embed a MailChimp pop up into my wp site using the following code, the site is https://jm-fit.com the actual pop up appears, but when you come to add your details is states there are errors, even though all fields are correctly filled out. Any suggestions?

Hi, I am using the SiteOrigin Unwind theme on my website, www.evancat.org. My site keeps crashing. At the moment, I’ve deactivated all plugins, except PageBuilder and SiteOrigin Widgets Bundle and WPforms Lite. I have a piece of Javascript to show reviews in my PageBuilder.
