Ninja Forms not displaying

Our Ninja forms are not displaying in 2 of the 3 places they are placed. It shows the title of the form, but nothing else.

Presentation Request Form

Workplace Training Request

(It is working fine here: https://namicentraltx.org/programvolunteerapp/)

I double-checked that “Automatically add paragraphs” is not checked.
I tried adding the shortcode for the form that IS working to one of the other pages, but it doesn’t show up in either place.

    Hi, I Work Here

    Hi, thanks for reaching out.

Have you made progress since posting? The forms on the two pages in question seem to be displaying normally on my end.

Well…. Sorta. The forms seem to only NOT work in the Austin (Texas?)
area. I only have a few data points, so it's hard to pinpoint where is displays and where it doesn't. When I remove the Date field from the form,
it then displays properly. None of it makes any sense at all. However, I think I can decidedly rule out it being an issue with SiteOrigin.

    Hi, I Work Here

Thanks for the update. I can only VPN in from Dallas, all looks normal. I’ll ask our developer Alex to take a look for us.

    Hi, I Work Here

    Hi,

I’ve had a look at the linked threads using a VPN based in Dallas and Austin and the forms worked on both pages for me. Hm. Have you tried clearing your browser cache on the computers still having issues? I ask as it’s possible the broken version of the form has been cached by the browser. If that doesn’t help, it could be specific to an ISP and that would explain why Andrew and myself aren’t able to replicate it when using a VPN (it’s unlikely the data centres that VPN use would be using the same ISP). Maybe ask around and see what ISP people who can’t see the form are using.

    Kind regards,
    Alex

It's definitely not a caching issue. When I remove the Date field from the form it works properly and if I add it back it stops working.

I've submitted a ticket with Ninja Forms as it seems that is where the issue must be.

    Thanks!
