Ninja Forms not displaying

Our Ninja forms are not displaying in 2 of the 3 places they are placed. It shows the title of the form, but nothing else. Presentation Request Form Workplace Training Request (It is working fine here: https://namicentraltx.org/programvolunteerapp/) I double-checked that “Automatically add paragraphs” is not checked.
