Shortcode not working in Page Builder

Wondering why the shortcode isn’t working when I’m trying to apply the form on the specific page of my choice? I disabled the automatic inclusion of forms on posts and pages and manually add form to the page of my choice but it still doesn’t work.

Need your advise. Thank you so much
