I am trying to put an RSVP form on http://justshabbat.org/index.php/rsvp/, and I can’t seem to find anything in the support forums regarding conflicts or problems with using the Google Forms plugin. However, my resulting form looks horrible, includes no tick boxes or multiple choice radio buttons. I’m not sure what’s going wrong.
I created my Google form in Drive.
I installed the Google Forms plugin.
I set up the basics of the form with no special features except for a custom confirmation URL.
Am I doing something wrong, or is this just not meant to be.

This could be a plugin conflict issue. Can you try disabling all non-SiteOrigin plugins and see if this fixes the issue? You’ll need to clear all your caches after disabling your plugins.
If it does fix the issue, then try re-enabling your plugins one by one until the issue comes back. This procedure will help diagnose which plugin is causing the issue.
Once we know that, we’ll be able to look at what might be causing the conflict and either solve the problem or help you find an alternative plugin.
If you aren’t using a SiteOrigin theme, then you can also try temporarily switching to one of the default WordPress themes to see if the issue is theme related.

To clarify, does this form still look like this when you change to a different theme? it’s quite possible it comes in an unstyled manner like this.
