page builder contact form module

I have been asked to make some changes to a site that uses page builder. It would appear Page builder has a built in contact form “module”? The client has added the built in contact form module from page builder to the main blog page in the page builder editor.
They want to display that same contact form on the single post pages. That would require adding the same contact form to a regular WP sidebar. There appears to be no contact form widget. Is there a way to add the contact form to a widget. I saw mention in the forums of a contact form widget in the site origins widget bundle but when I installed it there is not one nor is is it listed as included

    Hi Mark,

    To clarify, all widgets that make use of the SiteOrigin Widgets Bundle framework will gracefully degrade to a normal WordPress widget.

    The SiteOrigin Contact form is in the SiteOrigin Widgets Bundle. You’ll need to look for the SiteOrigin Contact form. If you can’t seem to find this widget please ensure it’s activated. You can check this by navigating to WP AdminPluginsSiteOrigin Widgets. If the SiteOrigin Widgets Bundle isn’t enabled at all, I’m not too sure what contact form you’re using but it’s not a SiteOrigin Contact form.
