Hopefully you’ve been able to find a solution.
You’ll need to choose between a hosted service like Poll Daddy: https://polldaddy.com/ or a plugin solution: https://wordpress.org/plugins/search.php?q=survey.
We unfortunately don’t have a recommend solution at the moment. Hope you’re able to find one.
