Do you have any idea why this form isn’t functioning?

Any idea why?
When it didn’t work the first time, I placed that small script from the HEAD in header.php. But it still didn’t work, so i’m just not sure why one form works and the other doesn’t.
I turned html compression off and tested that as well with no luck.
Any assistance is greatly appreciated!

Can you try disabling all non-SiteOrigin plugins other than the one your are using for the form and see if this fixes the issue? If it does fix the issue, then try re-enabling your plugins one by one until the issue comes back. This procedure will help diagnose which plugin is causing the issue.
About the order number:
If you are running Vantage Free on your live site the easiest way to upgrade would be via Appearance > Vantage Premium, click Already Paid? top right, enter your order number and follow the prompts to upgrade.
You should be able to tell now which version you’re running by going to Appearance > Themes, if you’re using Vantage Premium the screenshot will say “Vantage Premium” on it.
Alternatively, you can switch to another theme, delete Vantage, and install the premium ZIP file from Appearance > Themes > Add New: Upload Theme. Once activated, go to Appearance > Theme Settings > Premium and enter your order number to ensure updates are delivered.
