Issue with contact form as a widget

I’ve created a contact form in WP and assigned it to a widget that should pull through on every page. In development, the form looks correct, but looking at the page in live mode, the form doesn’t pull through correctly and I cannot figure out why? Website is www.stablewoodcoastalcottages.com

Can anyone help?

    Thanks for reaching out.

    Which plugin are you using to create the form? How are you outputting the form, using a widget from the contact form author? It can be worth trying to output the form via a shortcode in a Text widget to see if that changes the output.
