    Hi, thanks for reaching out.

    At the moment, the Contact Form widget, unfortunately, doesn’t offer an HTML or general content field. Would the image be presented to provide information?

    Thanks for the update. At the moment, it isn’t possible to insert HTML or text into a SiteOrigin Contact Form. You could perhaps use https://wordpress.org/plugins/contact-form-7/. You can insert an image into a form using an img tag as follows https://www.w3schools.com/tags/tag_img.asp.
