Gravity Form check boxes will not align correctly

Please scroll mid way down the page and view the check boxes below the title/lable “WHICH TYPES OF INSURANCE DO YOU WANT TO RECEIVE A QUOTE ON?”

Notice the check boxes are above the label instead of aligned.

When I remove the side bar it corrects itself.
