Contact Form not displaying phone number

When I create a contact form the phone field is not displaying. I create the extra field and select input of phone number then add a description label for ‘Phone’, but nothing is displaying. Also, when I try to edit the widget for any of the premium plugins the browser defaults to the time and date field being displayed on a complete white screen inside the browser. Please help!?

    Hi Frank,

    Do you have a public URL where we can take a look at what’s going on?
    Would it be possible for you to send me an export for this widget? To do this please open up the page containing the Contact Form in the editor and click the Layout button in the Page Builder toolbar. Then click import/export and then click download. Please upload the export to a 3rd party file hosting site such as WeTransfer (no email is required).

    By the way, as you’re a premium user, you’re entitled to priority email support. If you would like to make use of that, please follow the instructions found on your Page: Dashboard (please refer to the support tab). Please reference this thread and include your SiteOrigin Premium License in your email.
