Contact form Blank fields after hitting submit/send button

Hi, I wonder why the Site origin contact form on my website provides me with blank fields after filling in the necessary info and hitting the send/submit button? I cannot find the answer so far. Could it be a caching problem? My website is https://www.senszu.nl/coaching-groningen/
