Checkbox & Radio option not working

Am trying to get a checkbox and/or radio button to work, somehow it won’t show on my page and I can’t figure out what’s wrong.

It’s supposed to show beside this text:
I agree to receive news and updates in accordance to the Terms of Service of this website.*

Also, if anyone knows how I can add a link to Terms of Service within this text that’d be great.
