Problem enabling/checking contact form checkbox

I have added a check box on a contact form on my site but you can’t see the checkbox just the descriptions?http://www.thisgirlcanorganise.com/contact-us/

    Hi Kelly,

    Have you added any options to your SiteOrigin Contact Form Checkboxes field? The Checkboxes field allows you to add multiple checkboxes to your contact form and each checkbox that is output is set up as a possible option.
