SiteOrigin Contact Form – how to set option of multiple answers

Hello! I really love SiteOrigin page builder and its widgets. I am hoping you can help me with the problem I have. I am using Contact Form and I’d like to put the posibility of choosing multiple answers when you click on radio buttons.

What can I do to achieve this? Its only for 2 questions on the form.
