I have paid for the theme but have not received an email or a code, I have also filled in the form for if I have not received an email within 15 minutes but still nothing. Is there something I am missing?

It’s possible that the email got lost somewhere alone the way. Sometimes our emails get caught in spam folders. Can you try using this form. Just enter your PayPal email address.
