simple-proxy issue

Hi. i downloaded and installed your “simple-proxy” plugin. config it all with your directions but after i save my settings my website is going look like this: http://sub.boxfa.me (plugin is enable at the moment) tested in 2 different hosting and one VPS . it’s all the same. please help me there i really need to…
